NDIS supports tailored to participant goals

Services are delivered across Ballarat, Victoria and surrounding suburbs. Supports are planned around the participant's NDIS plan, goals, risks, language needs, cultural preferences and available worker capacity.

Support with daily routines, personal care, meal preparation, household tasks and building independence at home.

Assistance to access the community, attend appointments, join activities, visit local services and maintain social connection.

Personal Care

Respectful assistance with personal routines, hygiene, dressing, mobility and daily living needs while promoting choice and dignity.

Community Nursing Support

Health-related support planning, nursing care and care coordination where appropriate to the participant's needs, funding and clinical instructions.

Behaviour Support Implementation

Support workers can follow authorised behaviour support strategies and positive behaviour support plans when provided.

Careful worker matching, risk assessment, communication and support planning for participants with higher support needs.

Supported Independent Living

Regular assistance with daily living tasks, routines, personal care, household skills and community access for participants living in shared arrangements.

Assistance to attend appointments, community activities, education, employment-related activities and other planned commitments.

Support to access or maintain employment-related, education, volunteering and community participation activities where included in the participant's plan.

Support to participate in group activities, centre-based programs and social opportunities that match the participant's interests and goals.

Easy Read and Language Support

Support to understand service information through plain language, Easy Read documents and interpreter support where required.

How services begin

Before support starts, we discuss goals, funded supports, routines, risks, preferences, cultural needs, communication needs and any clinical or behaviour support instructions already in place. For SIL enquiries, we also discuss shared living routines, overnight support needs and compatibility with the household environment.

Helpful information for referrals

  • Participant name, age and preferred contact person.
  • NDIS plan dates, funding type and support categories.
  • Support needs, preferred days and location.
  • Preferred language, interpreter needs or Easy Read requirements.
  • Any behaviour support plan, risk plan or health care instructions.

Inclusive services

Support that respects culture and communication needs

Participants and families should be able to understand services before they agree to them. We ask about preferred language, communication style, cultural needs and decision-making supports during intake and review.

We can provide or arrange

  • Plain language explanations of services and fees.
  • Easy Read style documents for key participant information.
  • Interpreter support for important conversations where required.
  • Respectful support for CALD participants and families.
  • Time for questions before service agreements are signed.
